chore(extraction): prototype MedGemma 27B normalization pass
Standalone tsx script piping synthetic page-text through MedGemma 27B-IT via LM Studio's OpenAI-compatible endpoint, mirroring the medMalExtractor prompt shape. Validates clinical-field quality and JSON/shape stability ahead of deciding whether to wire it in as a stage-2 normalization model.
