docs: add med-mal project templates plan
Plan for a "Medical Malpractice Case" project template that scaffolds the standard subfolder structure on project creation and registers reusable tabular-review column schemas (chronology, bills, MAR, transfusion log, vitals, labs, imaging index, red-flag scan) the user can instantiate once documents are uploaded.
