Revise receipts always run the structural lint; count reasonless edits
The determinism plan's flagship hook - deterministic same-turn feedback on every library_revise_docx receipt - was gated behind the opt-in annotate flag, so ordinary redlines got none. Lint runs unconditionally now, and edits_without_reason is counted on every revision so rationale coverage is a measurable variable (annotate mode still forces it to zero by rejecting reason-free edits).
