Stream claude-p transport with inactivity watchdog and effort
stream-json + --include-partial-messages keeps bytes flowing during long single-shot generations, so the watchdog kills on 240s of silence rather than total wall time (whole-document turns legitimately run 10-30 min; hard cap 3600s). params.reasoningEffort now passes through as --effort. tsc clean; live smoke green.
