docs: adopt the SourceDoc consolidation as P1.1a and index the eval plan
P1.1 will be executed as one consolidation: a single content-hashed SourceDoc per fetched source, providers as compilers, consumers as queries, each cutover gated on byte-identical parity over the live-probed fixture corpus.
