claude-p watchdog: liveness counts model stream events only
Same mechanism as the LAB adapter fix: init lines print at spawn and were flipping the activity flag, so the 240s clock killed first calls still in prompt/cache processing (>240s TTFT on Beaver-sized contexts, probe-confirmed no rate-limit queueing). 900s grace to the first model event, 240s inactivity after.
